It’s for a Disability class, and my subject is about ADDICTION as a disability. (it’s apart of the ADA)
1. Has your drug and/or alcohol use ever prevented you from being able to show up for work more than 3-4 x’s in a 3 mo. period?
2. Has the ability to accuratley perform your job tasks decreased at times because of your use?
3. Have you ever wanted to stop or decrease your use so you can do the things you used to do before your use?
4. When you use do you isolate?
5. Do you consider yourself to be more active in your own life and decision making when you’re not using?
6. Does your use effect your relationships with your friends/family?
7. In life in general have you ever changed your goals to meet your behaviors instead of changing your behaviors to meet your goals?
8.Do you have a co-occuring diagnosis? If so, do your symptoms worsen when you use?

Is this just a survey? I don’t mean to be condescending but you are not going to get much more than a cursory response. Yahoo answers is great but it would be impossible to answer these questions in enough detail. If it is just a survey, why not just make them up? You would learn just as much.

But if you are serious about the topic then I would suggest that you come up w/ better questions. These are all yes/no questions except for the last one. I’ve had two DUII’s and this looks a lot like an intake questionnaire.
